Pre-Placement Health Assessment

If you are a contractor providing services for which pre-placement health assessments are required, please contact TMS on 0118 982 4537 to discuss the arrangements.

In preparation for the assessment, details of the work to be done should be forwarded to TMS on a 'Job Hazard Identification Form' (TMS Form F154). Each employee should also complete an 'Employee Health Declaration Form' (TMS Form F115).

Please note that drug and alcohol testing will be included in pre-placement assessments unless alternative arrangements are agreed.
